Care Management

Under the Support at Home Program, you’ll be supported by a dedicated Care Partner who will work with you to design a personalised care plan based on your needs, goals, and interests.

Your Care Partner will:

• Take the time to understand what matters most to you
• Help you choose the right services and supports
• Consult with your nominated supporter (if you wish)
• Update your care plan and budget as your needs change
• Check in regularly to ensure your care continues to meet your goals

For ongoing services, the Australian Government automatically allocates 10% of your quarterly budget to care management.

Contribution Rates and Fees

Contribution Rates

Your contribution rate is determined by the Australian Government (Services Australia).
It is based on:

• The type of service you receive
• Your pension status
• Your income and assets assessment

You’ll be required to pay contributions at your assessed rate for services under the Everyday Living and Independence categories.
No contribution is required for services in the Clinical category.

Self-Management – Choose Your Own Supplier

You have the flexibility to choose and manage your own services, with support from your Care Partner.

• You may select from our range of trusted contractors, or choose your own supplier.
• If you choose your own supplier, any related charges will be clearly outlined in your budget.

Assistive Technology and Home Modifications

We can assist in organising equipment, assistive technology, and home modifications to help maintain your comfort and safety at home.

Assistive Technology Administration Fee:

• 10% of the total cost (or bundle)
• Capped at $500 (whichever is lower)

Home Modifications Coordination Fee:

• 15% of the total quoted modification cost
• Capped at $1,500 (whichever is lower)

Our Care Partners will work with you to ensure a smooth and coordinated process, confirming and agreeing on all prices before any purchases are made.

Please Note:

• All listed prices are exclusive of GST.
• A minimum of 24 hours’ notice is required for cancellations. Cancellations made with less than 24 hours’ notice will incur a full service fee.
• A call-out fee for nursing services will be charged at 1 hour of the applicable RN or EN rate, in addition to the standard rate for any staff provided.
• Nursing initial visits are charged for a minimum of 1 hour, or longer depending on the complexity of the case. This includes time required for initial assessment, care plan formulation, product ordering, and attending to care needs.
• Travel costs are charged at $1.30 per kilometre, with additional charges for parking and tolls where applicable.

*Inactive sleepover shift – These are charged for a continuous shift of 9-hours between the hours of 10pm and 7am. This shift assumes that staff will be provided with private space which includes a bed.
